The performance lab needed a single entry point for new athletes and coaching staff. Before this portal existed, intake data lived in spreadsheets, email threads, and paper forms. The goal was to build one structured flow that captured movement history, training background, and current injury status before the first assessment session.
We mapped the intake process from first contact to the initial biomechanical screening. The portal was designed around three stages: athlete profile, training log, and screening consent. Each stage was kept short on purpose, so a coach could complete it between sessions without losing context. Field labels used plain language, and every section explained why the data mattered for the assessment.
The portal was built as a lightweight web form with role-based access. Coaches could pre-fill team rosters, while individual athletes submitted their own movement history. The screening consent step was integrated directly into the flow, replacing the old paper waiver. Data validation flagged incomplete fields before submission, reducing follow-up emails by a noticeable margin.
The first full season with the portal showed a measurable drop in intake errors. Assessment sessions started on time because screening data was already in the system. Coaches reported that the structured format made it easier to spot recurring movement complaints across their roster. The portal became the default starting point for every new athlete evaluation.
